Nurses Health Corner

WELCOME BACK PARENTS AND STUDENTS ~ SCHOOL YEAR 2024-2025!

We hope you and your families are well.  We are happy to welcome back our students.  We need your help to keep our school healthy.

Any student who is sick is expected to stay home (temperature of 100 or more, vomited, diarrhea, pink eye, lice, new rash). 

Any student who becomes sick or seriously injured while at school must be picked up immediately (may not ride the bus home). 

Students may not return to school until symptoms have improved for at least 24 hours (off of fever reducing medicine). 

Any student who is prescribed antibiotics must complete at least 24 hours before returning.

Any student who is sick for 3 or more days must return to school with a note from a health care provider.

Any student who is seen in urgent care or the emergency department must have a note.

First Philadelphia Prep Charter School must follow the guidelines set by the Pennsylvania Department of Health regarding health reports, immunizations, and mandated health screenings.  Below is a list of documents you need to send to the school nurse as soon as possible, if not already submitted.  Providing the school nurses with these reports allows the school nurses to identify any immediate health needs that need to be addressed for your child’s health needs during the new school year.

  • Physical exam report  (on or after 7/1/23) is required for all new students and for grades K, 6, and 11.
  • Dental exam report (on or after 7/1/23) is required for all new students and for grades K, 3, and 7.
  • Immunization record *All students must be up to date with immunizations by the first day of school.*
  • Student Medical Health Concerns Sheet for School Year 2024-2025
  • MED-1 form (if the student requires scheduled medication in school). ***All medications must be brought to school
    by a parent or guardian in the original package from the pharmacy with the MED-1 signed by the medical provider and parent***

These forms can be found on our school website via the health forms link under the parent tab, Nurse’s Corner tab.  If you have already submitted these forms, there is no need to resubmit them.  Letters will be sent out to all students after review of student health files if there is missing paperwork. If you have any questions or concerns please contact the school nurse.
